Why would an older small breed dog become obsessive about hygiene?

I have no idea what behaviors you think are indicative a very human mental state, but guess it has something to do with licking/chewing?

(1) My dog has licked her back left foot raw and only that foot.

Licking and chewing are what a dog does when something doesn’t feel right - it itches, it’s in pain, its swollen - or when they are very anxious - canine cognitive decline, loss of a sense like sight or hearing, generalized pain in some other part of the body.

(3) There is a small lump near my dog’s anus and he chews on it several times a day to the point that the fur is gone.
